⚠️ Cancellation Policy:

• 7-day written notice required before next billing date
• No prorated refunds for unused time
• Canceling forfeits any promotional or locked-in pricing

• You must be 18 years or older to enroll in a membership
• Clients under 18 must have a parent or legal guardian sign a consent form in person before using any tanning equipment
• Parental consent must be on file prior to any session

📝 Tanning Acknowledgment & Waiver

By enrolling in this membership and/or using tanning services, you acknowledge and agree that:

• You understand that UV tanning exposure may increase the risk of skin damage, premature aging, and skin cancer
• You agree to follow all recommended exposure times based on your skin type
• First-time clients may have session times adjusted based on skin type and staff recommendation
• You understand that overexposure may result in burns or injury
• You agree to wear appropriate protective eyewear during all tanning sessions
• Certain medications or medical conditions may increase sensitivity to UV exposure; you agree to consult a physician if unsure
• Individual tanning results may vary
• You release and hold harmless MAAC on Main from liability related to misuse, overexposure, or failure to follow guidelines

⚠️ Additional Terms:

• Memberships are non-transferable
• A valid payment method must be kept on file
• Declined or failed payments may result in suspension of membership access
• MAAC on Main reserves the right to refuse service for safety, health, or policy violations
